Indications

  • Dabigatran is prescribed to prevent stroke and harmful blood clots, particularly in individuals with a specific type of irregular heartbeat known as atrial fibrillation.
  • It is also employed in the treatment of blood clots in the veins of the legs (deep vein thrombosis) or lungs (pulmonary embolism), and to diminish the risk of their recurrence.
  • Additionally, this medication may be used as a preventive measure against blood clots after hip replacement surgery.
  • Dabigatran functions as an anticoagulant by blocking a clotting protein called thrombin in the blood, promoting smooth blood flow within the body.
  • It is important to note that dabigatran is not suitable for preventing blood clots after artificial heart valve replacement. Individuals who have undergone heart valve surgery should consult their doctor to determine the most appropriate medication.
  • Patients are advised not to discontinue any medication, including dabigatran, without consulting their doctor first.

Dosage and administration

  • Before starting dabigatran and with each refill, carefully read the Medication Guide and, if available, the Patient Information Leaflet provided by your pharmacist. If you have any questions, consult your doctor or pharmacist.
  • Take this medication orally, with or without food, as prescribed by your doctor, typically twice a day. For children, the initial dose should be taken in the morning and the second in the evening, approximately 12 hours apart. After hip or knee replacement surgery, follow your doctor’s instructions, usually taking it once a day. Avoid antacids within 24 hours after surgery to ensure optimal effectiveness. If you experience stomach upset, take it with food.
  • Swallow the capsules whole with a full glass of water (8 ounces/240 milliliters). Do not crush, chew, or break open the capsules, as this can release the entire dose at once, increasing the risk of side effects. Keep the medication in its original bottle (or blister package) to protect it from moisture. Refrain from using a pill box or medication reminder box.
  • The dosage is determined based on your medical condition, kidney function, response to treatment, and other medications you may be taking. Inform your doctor and pharmacist about all products you use, including prescription drugs, nonprescription drugs, and herbal products. For children, the dosage is also influenced by age and weight.
  • To maximize the medication’s benefits, take it regularly at the same time(s) each day and adhere strictly to the prescribed dosage and schedule.
  • It is crucial to follow the prescribed instructions precisely. Do not increase your dose or take the drug more frequently than directed. Consult your doctor before discontinuing the medication.

Side effects

  • Easy bruising, minor bleeding (such as nosebleeds or cuts), upset stomach, or heartburn may occur with this medication. If these effects persist or worsen, inform your doctor or pharmacist promptly.
  • Remember that your doctor prescribed this medication because they assessed that the benefits outweigh the risks of side effects. Many individuals using this medication do not experience serious side effects.
  • Serious bleeding can occur if this medication excessively affects your blood clotting proteins. Contact your doctor immediately if you observe signs of serious bleeding, such as frequent or uncontrolled nosebleeds, unusual tiredness, weakness, pain, swelling, discomfort, easy bruising, prolonged bleeding from cuts or gums, heavy or prolonged menstrual flow, pink or dark urine, coughing up blood, bloody or coffee ground-like vomit, severe headache, dizziness, fainting, or bloody/black/tarry stools.
  • Seek immediate medical assistance if you notice signs of very serious bleeding, including vision changes, confusion, trouble speaking, or weakness on one side of the body.
  • While rare, a severe allergic reaction to this drug can occur. Seek medical help promptly if you experience symptoms such as rash, itching, swelling (especially of the face, tongue, or throat), severe dizziness, or difficulty breathing.
  • This list does not encompass all possible side effects. If you notice any other effects not mentioned here, contact your doctor or pharmacist.

Precautions

  • Prior to taking dabigatran, inform your doctor or pharmacist about any allergies you may have. This product may contain inactive ingredients that could trigger allergic reactions or other issues. Seek details from your pharmacist regarding these inactive ingredients.
  • Before using this medication, disclose your medical history to your doctor or pharmacist, especially if you have a mechanical heart valve, kidney disease, bleeding problems (such as gastrointestinal or brain bleeding), blood disorders (e.g., anemia, hemophilia, thrombocytopenia), liver disease, recent major injury or surgery, history of stroke, a specific clotting disorder (antiphospholipid syndrome), or a tendency for frequent falls or injuries.
  • Prior to undergoing surgery or any medical/dental procedures, especially those involving spinal puncture or spinal/epidural anesthesia, notify your doctor or dentist about your use of this medication and provide information about all the products you use, including prescription drugs, nonprescription drugs, and herbal products. Your healthcare provider may instruct you to stop taking dabigatran before the procedure, so seek specific guidance on stopping or resuming this medication.
  • Stomach bleeding may result from the use of this medication. Avoid daily alcohol consumption, as it may increase the risk of stomach bleeding. Consult your doctor or pharmacist for guidance on the safe amount of alcohol you may consume.
  • Exercise caution with sharp objects like razors and nail cutters, and be mindful of activities such as contact sports, as this medication can cause heavy bleeding. Use an electric razor for shaving and a soft toothbrush for dental care. In case of falls or injuries, especially head injuries, contact your doctor promptly for evaluation.
  • Older adults may have a higher risk of bleeding when using this drug.
  • If you are pregnant, use this medication only if clearly necessary, and discuss the potential risks and benefits with your doctor.
  • Dabigatran passes into breast milk, so consult your doctor before breastfeeding while using this medication.
