Dosage and administration

  • Before starting nortriptyline, read the Medication Guide provided by your pharmacist. If you have any questions, consult your doctor or pharmacist.
  • Take this medication orally as directed by your doctor, usually 1 to 4 times daily. If using the liquid form, measure the dose carefully with a special measuring device/spoon. Do not use a household spoon as it may result in an incorrect dose.
  • Dosage is determined based on your medical condition and response to treatment. To minimize the risk of side effects (like dry mouth, and dizziness), your doctor may initiate treatment with a low dose and gradually increase it. Follow your doctor’s instructions carefully.
  • Take this medication regularly to maximize its benefits. To help remember, take it at the same time(s) daily. Do not increase the dose or frequency of use beyond what is prescribed. Doing so won’t accelerate improvement and can heighten the risk of side effects.
  • Continue taking nortriptyline even if you feel well. Do not discontinue without consulting your doctor, as certain conditions may worsen with sudden cessation. Abrupt discontinuation may result in symptoms such as mood swings, headaches, fatigue, and changes in sleep patterns. To minimize these symptoms, your doctor may taper your dose gradually. Seek advice from your doctor or pharmacist for more information. Report any new or worsening symptoms promptly.
  • Nortriptyline may not produce immediate effects. Although some benefit may be observed within a week, it may take up to four weeks to feel the full effect.

Side effects

  • Common side effects of this medication may include drowsiness, dizziness, dry mouth, blurred vision, constipation, weight gain, or difficulty urinating. If any of these effects persist or worsen, notify your doctor or pharmacist promptly.
  • To reduce the risk of dizziness and lightheadedness, rise slowly from a sitting or lying position.
  • To alleviate dry mouth, consider sucking on sugarless hard candy or ice chips, chewing sugarless gum, drinking water, or using a saliva substitute.
  • To prevent constipation, incorporate dietary fibre, ensure adequate hydration, and engage in regular exercise. You may also need to take a laxative. Consult your pharmacist to determine which type of laxative is suitable for you.
  • Remember that this medication has been prescribed because your doctor has determined that the benefits outweigh the risks of side effects. Many individuals using this medication do not experience serious side effects.
  • Inform your doctor immediately if you experience any serious side effects, including persistent heartburn, tremors, mask-like facial expressions, muscle spasms, severe abdominal pain, decreased sexual ability or desire, or enlarged or painful breasts.
  • This medication may increase serotonin levels and rarely lead to a serious condition called serotonin syndrome/toxicity. The risk is higher if you are also taking other medications that increase serotonin. Inform your doctor or pharmacist about all the medications you are taking. Seek medical assistance immediately if you develop symptoms such as rapid heartbeat, hallucinations, loss of coordination, severe dizziness, severe nausea, vomiting, diarrhea, muscle twitching, unexplained fever, or unusual agitation/restlessness.
  • Seek immediate medical help if you experience very serious side effects, including severe dizziness, fast or irregular heartbeat, fainting, seizures, eye pain, swelling, redness, widened pupils, and vision changes (such as seeing rainbows around lights at night).
  • While very rare, a serious allergic reaction to this drug can occur. Seek immediate medical help if you notice symptoms of a serious allergic reaction, including rash, itching, swelling (especially of the face, tongue, or throat), severe dizziness, or difficulty breathing.

Precautions

  • Before taking nortriptyline, inform your doctor or pharmacist if you are allergic to it, other tricyclic antidepressants (such as amitriptyline), or if you have any other allergies. This product may contain inactive ingredients that could trigger allergic reactions or other complications. Discuss this with your pharmacist for more details.
  • Prior to using this medication, disclose your medical history to your doctor or pharmacist, especially if you have breathing problems, liver problems, recent heart attack, difficulties urinating (such as due to an enlarged prostate), overactive thyroid (hyperthyroidism), personal or family history of glaucoma (angle-closure type), personal or family history of mental/mood conditions (such as bipolar disorder, psychosis), family history of suicide, seizures, or conditions that may increase your risk of seizures (such as other brain diseases or alcohol withdrawal).
  • Nortriptyline may cause a condition that affects heart rhythm (QT prolongation), which can rarely result in serious (rarely fatal) fast/irregular heartbeat and other symptoms (like severe dizziness, and fainting) requiring immediate medical attention.
  • The risk of QT prolongation may be elevated if you have certain medical conditions or are taking other drugs that may cause QT prolongation. Before using nortriptyline, inform your doctor or pharmacist of all the drugs you take and whether you have certain heart problems, a family history of certain heart problems, or low levels of potassium or magnesium in the blood.
  • This medication may cause dizziness, drowsiness, or blurred vision. Alcohol or marijuana (cannabis) can increase these effects. Avoid driving, using machinery, or engaging in activities requiring alertness or clear vision until you can do so safely. Limit alcoholic beverages. Discuss marijuana use with your doctor.
  • Before undergoing surgery, inform your doctor or dentist about all the products you use, including prescription drugs, nonprescription drugs, and herbal products.
  • This medication may increase sensitivity to the sun. Limit sun exposure, avoid tanning booths and sunlamps, use sunscreen, and wear protective clothing outdoors. Notify your doctor if you experience sunburn or skin blisters/redness.
  • The liquid form of this medication may contain alcohol. Use caution if you have diabetes, alcohol dependence, liver disease, or any other condition requiring you to limit/avoid alcohol in your diet. Consult your doctor or pharmacist about using this product safely.
  • If you have diabetes, this drug may affect blood sugar control. Monitor your blood sugar levels regularly and inform your doctor of the results. Your doctor may need to adjust your diabetes medication, exercise program, or diet.
  • Older adults may be more sensitive to the side effects of this drug, particularly dry mouth, dizziness, confusion, difficulty urinating, and QT prolongation.
  • During pregnancy, use this medication only when clearly needed. Untreated mental/mood problems (such as depression, anxiety, and panic disorder) can be serious, so do not stop using this medication unless directed by your doctor. If planning pregnancy, becoming pregnant, or suspect pregnancy, discuss with your doctor the benefits and risks of using this medication during pregnancy.
  • This medication passes into breast milk and may have undesirable effects on a nursing infant. Consult your doctor before breastfeeding.
